- Direct Deposit Information
- Voided check or letter from your bank on their letterhead.
- Letter from your bank must have account number, routing number, and be signed by bank representative.
- If you do not wish to enroll in direct deposit, your pay will be loaded to a WV Pay Card.
- Insurance Benefit Selections
- Names of plans selected (for health, dental, vision, additional benefits)
- Birth and marriages certificates for spouse and children you wish to add to your coverage.
- Social security number and birth date for your spouse and children you wish to cover.
- Consolidated Public Retirement (PERS)
- Name, address, social security number, and date of birth of your beneficiary.
